Observation

The provider self-reported a brief lapse in supervision. Upon being made aware of the incident, the provider followed up with staff, the parent of the child, and the licensor. In response to the incident, the provider met with staff to review expectations for tracking children. Staff are expected to count children throughout the day and before and after each transition of staff or change in location for children. The facility also uses an app that staff will use to account for children before and after each transition of staff or change in location for children.
